Patient Bill of Rights

  1. Patient has the right to considerate and respectful care.

  2. He has the right to obtain from us complete and current information concerning his diagnosis, treatment, and prognosis in terminology the patient can be expected to understand.

  3. Patient has the right to receive from us information necessary to give informed consent prior to the start of any procedure and/or treatment…, and where medically significant alternatives for treatment or care exist, or when the patient requests information concerning medical alternatives, the patient has the right to such information, and to know the name of the person responsible for the procedures and/or treatment.

  4. He has the right to refuse treatment to the extent permitted by law, and to be informed of the medical consequences of his action.

  5. Patient has the right to every consideration of his privacy concerning his own medical care program.

  6. He has the right to expect that all communications and records and care are treated as confidential.

  7. He has the right to expect us to expect that, within our capacity, our facility should make a reasonable response to the request of any patient for services, provided such patient clearly demonstrates his ability to pay for the services rendered – with a proof of a confirmed (and accepted) method of payment.

  8. Patient has the right to obtain his information concerning any relationship in this Facility to other healthcare facilities – for continuous care, including any professional relationships among individuals, by name, who are treating him except in Personal Injury Cases.  In cases of Personal Injury, no such Records shall be released to any Patient without a proven signature on our “Lien Agreement” from his Attorney or Insurance Company, except following proof of  full payment of medical bills.

  9. He has the right to be informed of any possible experimentation that may affect his care or treatment, as well as the right of refusal to participate.

  10. Our Patient has the right to examine and receive an explanation of his bill, regardless of the source of payment.
